Our Services
Our Support is tailored to each young person’s individual needs, through a structured care and support plan, which uses the Young Person’s Star model to encourage each person to take control of their lives and work towards achieving their personal goals. Support is focused on eight areas:
- Accommodation
- Work And Learning
- People And Support
- Health
- How You Feel
- Choices And Behaviour
- Money And Rent
- Independent Life Skills
Key worker
Each person will be allocated a key worker who will work closely with the young person and their family members, taking time to get to know everyone and understand what is working well and what personalised support may be required.
The Keyworker will:
Supports the young person and their family to set targets for goals the young person would like to achieve
- Ensure the right support is given at the right time and place
- Ensure that the voices of the young person and their family members are heard always.
- Communicate with the team the young person and his families preferences
Key working ensures that local systems are able to meet the needs of children and families, which means they get the right
support at the right time and their needs are met in a comprehensive and coordinated way.
We work closely with any individual providing or coordinating a specific treatment or package of therapeutic care and support to attend CAMHS or adult mental health services to the young person.
We proactively facilitate a young person’s access to the range of therapeutic resources. In addition to the services outlined above, we have established positive working relationships with a variety of other agencies and individuals. These can be accessed in consultation with the placing authority to address the young person’s needs for such issues as sexual or physical abuse, drug and substance abuse, bereavement, and loss.
